Senior Healthcare Associates Replacement Guarantee (Effective January 2007)

You will not be required to utilize all of our services to qualify for our replacement guarantee

Lifetime Replacement Guarantee for Hearing Aids - Senior Healthcare Associates will fit and dispense replacement hearing aid(s) at no cost to the patient, the patient's family or the facility, if a patient's hearing aid, dispensed through Senior Healthcare Associates, is lost or broken beyond repair as long as the patient is a current resident of said contracted facility.

Lifetime Replacement Guarantee for Eyeglasses - Senior Healthcare Associates will provide replacement at no cost to the patient, the patient's family or the facility, if a patient's eyeglasses are lost or broken beyond repair as long as the glasses were dispensed through Senior Healthcare Associates and the patient is a current resident of said contracted facility.

Lifetime Replacement Guarantee for Diabetic Shoes - Senior Healthcare Associates will provide replacement at no cost to the patient, the patient's family, or the facility, if a patient 's diabetic shoes, dispensed through Senior Healthcare Associates are lost or worn beyond repair as long as the patient is a current resident of said contracted facility.

Lifetime Replacement Guarantee for Dentures* - Senior Healthcare Associates affiliate Dental Group will provide replacement at no cost to the patient, the patient's family, or the facility, if a patient's dentures dispensed through Senior Healthcare Associates affiliate Dental Group are lost or broken beyond repair as long as the patient is a current resident of said contracted facility.

* Service may not be available in all market areas
